Role of the granuloma in recurrent Crohn's disease.

چکیده
The clinical significance of sarcoid-like granulomata in recurrent Crohn's disease has been investigated in 90 patients. The recurrence rate for non-granulomatous disease was double that of granulomatous disease. In granulomatous disease, involvement of the mesenteric lymph nodes did not affect the recurrence rate. This suggests that sarcoid-like granulomata reflect the patients' immune response and support the view that excision of the lymphatic drainage en bloc with affected bowel is not indicated.

Genetic variants in autophagy-related genes and granuloma formation in a cohort of surgically treated Crohn's disease patients.
BACKGROUND AND AIMS Granulomas are a characteristic microscopic finding in Crohn's disease. Their clinical significance is controversial and their pathogenesis is unknown, but impaired processing of bacterial components has been suggested. Autophagy is a fundamental process involved in the elimination of intracellular bacteria. Genetic variants in autophagy genes IRGM and ATG16L1 have been asso...
متن کاملHLA B8 and granuloma formation in Crohn's disease.
In a retrospective survey, patients with Crohn's disease who were HLA B8 were found to have significantly fewer granulomas in rectal biopsies (p less than 0.002) and in resected bowel specimens (p less than 0.02) than non-HLA B8 patients. No statistically significant difference was found between HLA B12 and non-B12 patients. This in vivo evidence of differing immune response capabilities betwee...
متن کاملCrohn's disease presenting as life-threatening ileal bleeding.
We report a case of Crohn's disease in a 32-year old Saudi male. The disease presented with severe, life-threatening ileal bleeding necessitating an urgent laparotomy and 100 cm of ileum and ascending colon was resected. The bleeding source was several ulcers in an inflamed ileum and histopathologic examination revealed typical findings of Crohn's disease with a chronic, transmural inflammation...
متن کاملInvestigation of adherent-invasive E. coli in patients with Crohn\'s disease
Background: Crohn's disease and Ulcerative colitis are known as inflammatory bowel disease with high morbidity which are as a result of increasing immune responses to intestinal microbiota in genetically susceptible individuals. The association of adherent invasive Escherichia coli with Crohn's disease in human has been discussed for decades. The principal aim of this study was to assess the re...
متن کاملMetastatic Crohn’s disease despite infliximab therapy*
Metastatic Crohn's disease is a rare extraintestinal manifestation of Crohn's disease. It is characterized by polymorphic skin lesions formed by non-caseating granulomas located on anatomical sites distant from the gastrointestinal tract. We report a rare case of metastatic Crohn's disease, simultaneously displaying multiple clinically heterogeneous cutaneous lesions, in a patient with previous...
